The Harrow Urgent Community Response team is an unique service that aims at providing a high quality clinical service in the patient's own home and preventing unwarranted admissions to hospitals.

Main duties of the job

• To provide skilled nursing care working under the supervision of Senior Urgent Community Response Clinicians to patients in a variety of community settings and hospital A&E.
• The post holder will undertake Rapid Response assessment duties involving delivery of nursing services and holistic assessments of patient's health needs using a person‑centric approach.
