Job Description:
PURPOSE OF JOB: Assist with all aspects of delivery of care and support for patients in an outpatient setting including:
Assisting physicians and extenders (NP, PA's)
Patient assessments and monitoring
Patient education
Triage nursing (telephonic and in person)
Coumadin management
Operation of equipment
Chronic care management
Risk stratifying of patients to identify high risk/ high cost patients and their medical needs
Telephonic outreach to patients that have been discharged from the hospital, nursing home, emergency room and/or urgent care
Other duties as requested
