Job Requirements
Must have BLS certification (AHA required).
Must have relevant state licensure for Occupational Therapist in Kansas.
Completion of Ascension Skills Checklist within one year of start; updated annually for specialty.
Must provide professional credentials including Ascension Resume, Signed Assignment Agreement, Ascension Badge Photo, and Ascension Profile.
Must provide personal health history documentation including MMR, Fit Test, Hepatitis B, Influenza Vaccine (optional), Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire, Health Statement, 2 Step PPD or IGRA, Varicella, and Tdap.
