Summary:


  • Location: In-person visits in Northeast Ohio; Cleveland, Akron, Canton, Ohio

  • Work Mode: Hybrid (In-person and remote)

  • Shift: Mon-Fri 8a-5pm EST; 30 min lunch

  • Duration: 6 months with potential to extend based on need



Responsibilities:


  • Assist members in Northeast Ohio with SSDI applications through in-person interactions in SNFs

  • Gather information via interviewing, obtaining signatures, and visiting local SSA offices

  • Launch the expansion of SOAR across the state

  • Review SNF census, navigate internal systems, and gather medical records following detailed SOP for SOAR SSA

  • Communicate effectively with members, including those with language and speech difficulties
