Job Summary:
Provide comprehensive outpatient physical therapy services to adult, adolescent, and geriatric patients. Deliver personalized treatment plans to improve mobility, function, and pain management. Collaborate with healthcare teams to ensure quality patient care.
Responsibilities:

• Assess patient conditions through evaluation and screening methods, including fall risk assessments.
• Develop and implement individualized treatment plans targeting orthopedic and neuro conditions.
• Manage a caseload with a focus on specific diagnoses such as Achilles Tendonitis, ACL tears, and post-operative recovery.
• Educate patients on injury prevention, rehabilitation exercises, and post-treatment expectations.
• Document patient progress and treatment plans accurately in NextGen EHR system.
• Collaborate with healthcare team members to coordinate care and optimize patient outcomes.
