We are currently looking to appoint a Support Planner, which will be working under the general direction and supervision of the Business Support Planner Lead and to work as a member of there enablement and peripatetic service to ensure the efficient and appropriate day‑time delivery of service inclusive of reablement, long term care, extra care, nightcare and day services

What will be expected from me?

You will:

• Effectively deal with queries and complaints
• Able to communicate effectively both orally and in writing
• Ability to keep accurate records and provide management information as appropriate.
• Ability to use and maintain information technology systems. Individual time management and prioritising essential workloads
• Organisational and team working skills.
