The Substance Use Nurse II is an RN who practices as an integral member of an interdisciplinary, collaborative care team focused on providing accessible and site-based primary care services to individuals experiencing homelessness residing in recovery residences. The care team will be focused on expanding low-barrier buprenorphine treatment access and warm handoffs to longitudinal care. On-site clinical services will include RN-led wound care, complex care management, and harm reduction education, among other general primary care needs. The RN will be a champion for trauma-informed care transformation for people with substance use, unsheltered homelessness, and medical complexity.
