Responsibilities Comprehensive Nursing Assessment & Care Planning - Conduct holistic nursing assessments for clients in the home setting
- Develop, implement, and review individualised care plans based on clients’ medical, functional, psychosocial, and caregiving needs
- Monitor clients’ progress and evaluate effectiveness of care interventions
- Review and update care plans regularly or whenever clients’ conditions change
Clinical Nursing Care & Interventions Provide skilled nursing care and clinical procedures, including:
- Wound assessment and wound management
- Enteral feeding tube management and replacement
- Urinary catheter care and management
- Medication administration and management
- Chronic disease monitoring and nursing interventions
- Manage acute and chronic medical conditions in consultation with doctors and multidisciplinary healthcare teams
