Key Responsibilities:
Evaluate and diagnose speech, language, cognitive-communication, voice, fluency, and swallowing disorders; assess home environment and identify equipment needs. Develop and implement individualized, physician-approved plans of care with measurable therapy goals. Provide dysphagia therapy and recommend appropriate diet modifications in coordination with the care team. Recommend and train patients and caregivers on AAC devices and home exercise programs. Monitor progress, adjust the care plan as needed, and submit accurate clinical documentation per JCAHO standards. Collaborate with physicians, nurses, PT, OT, and other team members through care conferences and interdisciplinary planning. If you bring the following, we'd love to connect:
