Main duties of the job

In this rewarding role, you will

• Lead and support our Hospice at Home team to deliver highquality care in patients homes
• Manage referrals, coordinate care, and work closely with multidisciplinary partners
• Provide expert clinical leadership, assessment, and guidance to staff, patients, and families
• Help prevent unnecessary hospital admissions, enabling patients to be cared for at home
• Support service development, governance, staff training, and clinical standards
• Line manage HCA and RN teams, providing supervision, mentorship, and performance reviews
