Case Manager / Social Worker – MOKO Services

MOKO Services is seeking an experienced Registered Social Worker passionate about making a noticeable difference to Māori tangata whai i te ora and their whānau. The preferred candidate will work alongside our cultural team to enhance the wellness of tangata whai i te ora, implementing social work skills while collaborating with a multidisciplinary team committed to improving Māori health gains and professional development.

Responsibilities

• Provide mental health assessment, treatment, and safety planning for clients.

• Organise and prioritise workload, coordinate care across agencies and family/whānau.

• Engage in community networking and effective community outreach.
