Working hours:Mon to Thursday (8am to 6pm) / (830am -630pm) 
Friday (8am to 5pm) (830am -530pm) 
Job Responsibilities Develop individualise care plans for residents 
Execute the care plan and monitor progress 
Coordinate care for residents with relevant stakeholder 
Requirements Degree in Social Work 
Preferably experience in Social Service Sector or related field
