Job Scope - Clinical Aspects
- Manage enquiries and referrals
- Conduct assessment and home/site visits, and provide counselling, behavioural interventions, groupwork and support forresidents and their families
- Conduct comprehensive assessments of residents' care and financial needs, and support in the submission of HOMES means testing and transport subsidy applications as required
- Look after clients' physical and emotional welfare, identify anomalies and provide the appropriate intervention or service linkage
- Manage emergency response and crisis management with regards to residents' behaviour management with the multi-disciplinary team
- Develop, implement and review Individual Training Plans forresidents with the multi-disciplinary team
