**Job Description:**

This is an exciting opportunity to join our outpatient social work care management team and work with pediatric patients seen in rehab clinics (audiology, physical therapy, occupational therapy, and speech). This position will be based at Primary Children's Lehi campus seeing patients there and also providing support to those patients seen in other satellite locations. As part of this role you will additionally see patients at those local satellite locations a few times a month. This social worker will be part of a multidisciplinary team, working with pediatric patients and families to address the emotional and practical challenges related to their diagnosis.
