Liquid Personnel is recruiting an Adult Social Worker to join its client’s Learning Disabilities team, based in Tyneside.
What will your responsibilities be?
Carry out needs assessments: Undertake Care Act assessments to identify the support requirements of adults with learning disabilities.Develop and review care plans: Work collaboratively with individuals, families, and multi-disciplinary professionals to create tailored support packages that promote independence and well-being.Safeguard and advocate: Ensure adults with learning disabilities are protected from harm, while promoting their rights, choices, and access to services.
