**SUMMARY OF POSITION:**

Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from admission to discharge, utilizes appropriate criteria for inpatient care and plans timely discharges. Provides emotional support to patients and their support systems in managing complex medical conditions in an inpatient setting.
