Case Support & Management
  • Support Social Workers/ Case Managers in conducting assessments, maintaining case documentation and following up on care plans.
  • To accompany patients for external appointments (eg. Bank or HDB visits etc).
Administrative Support
  • Ensure timely and accurate updates of case notes and proficiency in MS Excel.
Volunteer Management
  • Plan and conduct volunteer activities for patients.
  • Coordinate with relevant teams to support implementation & execution.
Stakeholder Liaison
  • Collaborate with relevant agencies and families to coordinate care plans.
  • To participate in multidisciplinary team meetings.
Qualification
  • Diploma in Social Work or related field (Psychology/Sociology)
Requirements
  • Experience in community or healthcare services would be an added advantage.
