Job Purpose To support the Medical Social Work Department in providing emotional, social, and practical assistance to residents and their next-of-kin through referral management, administrative support, enquiry handling, basic supportive counselling, and coordination of social and financial assistance services, with the aim of enhancing coping, well-being, and quality of life.
Details and Responsibilities - Receive, process, and maintain referral records for nursing home and day care centre admissions
- Support Medical Social Workers (MSWs) in intake assessments, admission coordination, and service arrangements
