Job Description
As part of the CREST team, you will be responsible to:
Support clients with or who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources. Monitor and support clients in the community as assigned by the lead.Organizing and conducting outreach to increase mental health and dementia awareness and promote early identification.Work with regional health services, Hospitals, volunteers, grassroots, schools, internal organisation departments etc. on activities and programmes for the seniors.
