About the Role

The Sensory Social Worker will specialise in supporting adults with sensory loss, including Deaf, hearing loss, visual impairment and dual sensory loss. The role is based at the Hulme District Office and forms part of the Manchester Local Care Organisation’s Sensory Team.

Key responsibilities include:

• Providing person‑centred assessment, planning and delivery of support for citizens with sensory impairment.
• Co‑producing care plans with citizens, families and multidisciplinary partners, taking into account communication needs and the complexities of sensory loss.
• Monitoring, reviewing and evaluating support plans to achieve agreed outcomes.
• Safeguarding adults with sensory impairments, including risk assessment, investigation and development of protection plans.
