Job Description

The Senior Social Worker is responsible for providing comprehensive psychosocial assessment, intervention, and case management support to clients with complex social and healthcare needs within the community. The role supports clients and caregivers to age-in-place safely and with dignity through coordination of care, advocacy, counselling, and multidisciplinary collaboration.

You will need to

(i) Conduct comprehensive psychosocial assessments for clients and caregivers.

(ii) Develop and implement person-centred care plans based on clients' needs and goals.

(iii) Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.

(iv) Manage complex and high-risk cases independently.

(v) Monitor clients' progress and review intervention plans regularly.
