            Responsibilities  Conduct phone screenings to collect biodata and gather social information for medical social workers. Contact patients for appointment reminders and rescheduling. 
Accompany medical social workers on field visits or conduct them independently. 
Timely completion of case recordings. 
Liaise with external agencies. 
Transcribe applications onto online referral systems. 
Monitor and follow up on referral statuses. 
Handle external correspondences. 
Attend to Medical Social Services enquiry emails. 
Maintain work statistics and perform clerical duties.
