Key Responsibilities Build and grow relationships with hospitals, rehab centers, the VA and assisted living facilities Collaborate with hospital and rehab discharge planners, case managers, and physicians to identify patients suitable for home care. Provide education to patients and families about our home care services. Conduct clinical assessments to determine eligibility and service needs.
