Job Duties

You'll primarily work office based with some requirements for on-site work. You'll coordinate patient discharges from the hospital to home infusion settings and support ongoing patient care in both home and infusion suite environments.
+ At least three (3) years of RN work experience is required.
+ Case management experience, RN Home Health and IV therapy experience is preferred.
+ A typical schedule is part time, 20 hours weekly, 8:30 am to 5 pm, with rotating weekends, holidays, and some call
