Job Function:
Schedule: Hybrid | 3 days on-site 2 days remote | Monday to Friday | 5d x 8hrs flexible hours (8:30am-4:30pm, 9:00am-5:00pm, or 10:00am-6:00pm) Conduct preliminary in-home evaluations for consumers. Review assessments to determine if the correct conclusion for eligibility of services was reached by the nurse assessor. Collect data related to assessments and assessment outcomes.
