PeaceHealth is seeking a RN Home Care Case Manager - Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $47.01 - $74.50. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.
