JOB SUMMARY The RN Care Manager will effectively facilitate care for patients with elevated risk factors and/or chronic diseases through assessment, education, self-management support and timely healthcare delivery to achieve optimal health, quality, and financial outcomes. The RN Care Manager will contribute to improved quality of care through the efficient use of resources, thereby enhancing quality, cost-effective outcomes. They will also act as an advocate for the individual’s healthcare needs, and coordinate care to minimize the fragmentation of health care delivery. This position is committed to the constant pursuit of excellence in improving the health status of the community.
MINIMUM QUALIFICATIONS Professional & Technical Skills Home Health or Case Management experience preferred Education Graduate of an accredited School of Nursing. BSN required or obtained within 2 years of employment. License(s) or Certification(s)...
