Job Summary and Qualifications

You will collaborate with other healthcare providers, both internally and externally, in overseeing an ongoing comprehensive plan of care for a complex patient or groups of patients. You must anticipate consequences of altered physiologic homeostasis, utilize advanced skills to initiate early interventions and participates in identifying, planning/implementing changes to improve service operations and patient care outcomes.
