Job Description

Main Responsibilities of Rehabilitation Assistant :

1. To assist the people we work with to become more independent with their physical care, personal hygiene, daily life tasks and skills with a person centred approach.

2. To participate in the creation of a clinical led rehabilitation plan and its implementation, ensuring that their independence is preserved and developed as far as possible and affording appropriate levels of privacy and dignity.

3. To maximise the use of the environment and tools within the service to promote independence.

4. To participate in a multi disciplinary approach by offering rehabilitation focused feedback to aid clinical planning and reviews.

5. To be active participants in rehabilitation goal setting.
