The Registered Nurse II serves as a member of the Adult Community Clinical Services (ACCS) Integrated Team; provides behavioral and physical health monitoring and support; Medication Administration Program oversite, coordination, and education; completes PHAs; participates in the development of IAPs; provides trauma-informed, person centered community-based interventions that target skill development in the areas of Health & Wellness; conducts routine assessments and provides medical care as needed; ensures that appropriate medical services areaccessed by collaborating with care coordination entities.
