GENERAL SUMMARY:
Assesses and identifies patient needs after hours in accordance with the hospice concept of care. Coordinates with the Interdisciplinary Team to provide patient/family care. Maintains the continuity of care by ensuring that patient needs are met. May perform triage on call for the hospice program after hours as assigned, or provide home visits after hours as needed.
EDUCATION & EXPERIENCE:
+ Requires graduation from an accredited school of nursing, and current Michigan licensure as a Registered Nurse.
+ Requires a minimum of one year Hospice R.N. experience.
+ Requires an understanding and acceptance of the Hospice concept of care, and knowledge of normal and abnormal responses of patients and families to hospice care and terminal illness. Requires a valid driver's license and an acceptable driving record.
