Pride Health is seeking a Home Health/Hospice RN for one of our clients in TX! (Contract)



Coverage: Houston area



Schedule: Mon-Fri 8hr days, on-call 

Contract: 3-month contract 

Pay Rate: $48-53 per hour



Duties and Responsibilities

  • RN Case Manager-hospice

  • IDG Meetings

  • Hospice care

  • Disease Process Education, Medication Education, Picc Line Care and Education

  • Lpn And Aide Supervisory Visits

  • Team Case Conference Meetings.




Requirements and Qualifications


  • Current active RN license

  • BLS

  • Prior hospice experience

  • Homecare Homebase
