What will your responsibilities be?
Complete strengths‑based assessments and Care Act evaluations, ensuring timely, person‑centred support planning that promotes independence, wellbeing, and choice.Manage a varied and often complex caseload, including safeguarding enquiries, mental capacity assessments, and multi‑agency coordination to ensure safe and effective outcomes.Work collaboratively with health partners, community services, and families, facilitating smooth transitions, reviewing care packages, and addressing any changes in need promptly and professionally.
