Physical Therapy Position

Responsibilities:

• Demonstrated ability to evaluate and develop an individualized plan of care for patients establishing goals of treatment and a plan for discharge; taking into consideration the patients' needs and limitations.
• Demonstrates appropriate skill and care in the treatment and management of patients.
• Treats patient's disorders according to developed policies and procedures.
• Consults with attending physician concerning the patient's needs and limitations, goals, plan of care, progress, and discharge plans.
• Coordinates, delegates, and supervises responsibilities assigned to supportive staff (Physical Therapy Assistants, Rehab Techs, etc.).
• Provides written home care programs for patients to follow.
• Develops a teaching plan based on identified learning needs and assesses effectiveness of educational strategies; family and/or caregivers should be included in teaching,...
