Job Summary : Provide skilled therapy intervention and treatmentto patients in a home setting. Including, appropriate utilization of treatmentplans and assessment skills. Monitor carefully the patient status and progresstoward goals as well as compliance with physician's orders. Responsible foreducating patients and caregivers while preparing the patient/caregiver fordischarge as appropriate.
Qualifications/Educational Requirements :
1. Aperson who is licensed, if applicable, by the State in whichpracticing,unless licensure does not apply and meets one of the following requirements:
(a) (1)Graduated after successful completion of a Physical Therapist education programapproved by one of the following:
(i) TheCommission on Accreditation in Physical Therapy Education (CAPTE).
(ii) Successororganizations of CAPTE.
(iii) Aneducation program outside the United States determined to be substantiallyequivalent to Physical Therapist entry-le...
