The position involves providing one-on-one, skilled therapy in the comfort of the patient's home.
Primary Responsibilities:

• Conduct in-home physical therapy evaluations and develop individualized treatment plans
• Deliver skilled physical therapy services aimed at improving mobility, strength, balance, and safety
• Educate patients and caregivers on home exercise programs and injury prevention
