AuthoraCare Collective is currently seeking a PRN Physical Therapist Case Manager/PT Kids Path for ourHome Health department supporting the following counties: Alamance, Orange, Durham, Person, Chatham, Caswell, Rockingham, Guilford, Stokes, Forsyth, Davidson.
The Physical Therapist provides services to retore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease.Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions.
Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance. Establish and maintain therapeutic relatio...
