**EDUCATION & EXPERIENCE**

**Minimum Qualifications:**

+ Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
+ Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.

**Preferred Qualifications:**

+ Doctor of Pharmacy degree.
+ Completion of an ASHP accredited post-graduate year 2 residency program in Hematology/Oncology OR two years of experience in Hematology/Oncology.

**LICENSES, REGISTRATIONS OR CERTIFICATIONS**

**Required:**
