Primary Responsibilities - Adopts a patient-centered approach and principles of the social determinants of health for patient management.
- Provides nutrition assessment, nutrition support, counselling and eucation to patients and carers, individually or in groups (specialised discipline).
- Provides nutrition intervention based on various therapeutic diets through different modes of communication (eg. phone/virtual/home visit/centre visit consultation).
