**Job Summary and Qualifications**

**The Performance Improvement Specialist supports providers within the VCP network to include monitoring progress towards network goals, quality care gap management, practice transformation to improve process flows, transitions of care for identified populations, and care coordination to drive quality and cost outcomes for the attributed populations.**
