The RN Case Manager is responsible for managing the care of pediatric patients, according to their individual needs and care plan using a multidisciplinary method. The Registered Nurse facilitates referrals to primary providers, specialists, healthcare facilities, and community resources. The Registered Nurse is responsible for direct client care including a holistic approach to the care of children and their families, including educational, psychosocial, behavioral, physical, mental, and emotional care/needs of the child. The Registered Nurse will educate the client and family regarding immediate care needs and preventative behaviors.

Qualifications

• Graduate of an accredited school of nursing with current RN licensure eligible in the state of Kentucky.
