            Interventional Pain Management Physician Overview & Highlights Location: Phoenix, AZ (with 2 days/month in Casa Grande ASC)
Schedule: Clinic + procedural mix (1 2 days/week in ASC)
Compensation: $450,000 guaranteed base
Bonus Structure:40% of office collections
25% of ASC collections (after implant costs)
Benefits: Medical, Dental, Vision; 4 weeks PTO/Sick Time; $2,000 CME stipend; malpractice and license coverage
Sign-On / Relocation Bonus: Available; enhanced packages for physicians bringing an existing patient panel or practice
 Location & Logistics Work Environment: Established outpatient pain management practice with access to ASC
Coverage Model or Hours: Blend of clinic and procedural days; typically 1 2 surgery days...
