Job Description:
Canaide is currently seeking a highly professional On-site Patient Advocate in East Florida (Daytona Beach, Daytona, Orange City, New Smyrna, Palm Coast, Waterman and Deland). He/ She will act as the initial contact person to pursue the application processes of the various programs including but not limited to completing the application, following up to obtain all necessary documentation, & maintaining ongoing communication with the patient/guarantors, case managers and or doctors.
Responsibilities :
Visit patients in the hospital & bedside to pre-screen for qualifying criteria of various Federal & State Medicaid/Social Security Administration (SSA) & County / HCRA programs
Maintaining ongoing communication with various agencies throughout the application process
