**Description**

JOB SUMMARY

The Occupational Therapist, Home Health, evaluates, plans, and administers comprehensive occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician.

RESPONSIBILITIES AND SCOPE

+ Manages an occupational therapy caseload with efficiency and efficacy

+ Completes clinical documentation in the medical record per department guidelines and completes department records for billing and statistics. Performs accurate assessments and documentation for evaluations, OASIS, discharge, and regular visits.

+ Manages resources needed to perform OT treatment plan to accomplish the goals set for the patient. Provides, modifies, or discontinues occupational therapy based upon an ongoing assessment/reassessment of the patient’s clinical status in accordance with physician orders
