We are seeking experienced and motivated Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for patients throughout Oregon. This is an excellent opportunity for providers looking to supplement their income with a flexible, part-time schedule.
Key Responsibilities:
- Conduct comprehensive in-home Health Risk Assessments (HRAs)
- Identify and document acute and chronic medical conditions
- Obtain vital signs and calculate BMI
- Review medications and pharmacological therapy
- Perform a physical examination
- Evaluate patients' physical, mental, and social health needs
- Educate and counsel patients and their families on health conditions and preventative care
Schedule & Flexibility: - Part-time, flexible scheduling
- Set your own hours
- Visits available 7 days a week
- Minimum commitment of 8-10 hours per week
