**SUMMARY OF POSITION:**

Seeking a licensed and certified advanced practice provider with experience or strong interest in nephrology. Ideal candidate is patient-focused, detail-oriented, and comfortable managing chronic kidney disease, hypertension, and related conditions in an outpatient setting. Must work well in a collaborative team environment. Nephrology or internal medicine experience preferred but not mandatory.
