AuthoraCare Collective is currently seeking a Nurse Practitioner (Home Based Primary Care). This is a full-time position; Monday - Friday 8:00am - 5:00pm (office base will be High Point) with on-call responsibility.
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
The Nurse Practitioner is responsible for delivering comprehensive primary care, palliative care, and chronic disease management to patients in their homes. Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers.
