Position Information

Join the Nurse Led Outreach Team (NLOT) and play a vital role in improving the lives of seniors by delivering urgent mobile nursing services to residents in long‑term care homes. You will address emerging health concerns, reduce unplanned transfers to emergency departments and hospital admissions, support capacity building within LTC, and facilitate system flow between hospitals and LTC environments.

Rotation/Shifts

Days, 8‑hour shifts. (For information purposes only – the Hospital reserves the right to change.)

Basic Requirements

• Bachelor of Science in Nursing
• Master’s degree in Nursing or a related discipline
• Member of registered nurses association of Ontario
• Member of nurse practitioners association of Ontario
• Minimum of two (2) years recent clinical experience in an acute care setting working with medically complex older adults
