About the Role
The role of Lakes Needs Assessment Service Coordination (NASC) is to complete holistic assessments of the needs of our clients, and coordinate support services for older adults with health and disability related needs, and some younger people whose needs relate to a chronic long term health condition. We also work with clients who have recently discharged from hospital, as well as palliative care clients. Our aim is to support our clients to continue to live safely at home, or enable them to move into Residential Care if needed.
