The caregiver in this role will serve patients in the East area including Ashtabula, Beachwood, Cleveland, Euclid, Willoughby and Mentor.

Cleveland Clinic Homecare provides skilled nursing, social services and physical, occupational and speech therapy for patients returning home after a hospital stay, outpatient procedure or doctor's visit. This team continues Cleveland Clinic treatment at home and helps patients learn or re-learn any skills necessary to stay safe and independent at home. As a Licensed Practice Nurse, you will visit and provide care to approximately 6 patients each day in the comfort of their own homes.
